Lead capture Web application

This is a fairly large and sophisticated Web application I wrote with Brent Rieck. He did all the heavy lifting PHP stuff, I did all the functional and visual design. This is for the dotcom we ran together, Leadcleaner.com. Our clients are direct marketing firms and ad agencies who want to put up landing pages for DM campaigns. The software captures, qualifies, and reports responses, based on configurable business logic. Here are a few screen shots.
